This provides step-by-step instructions for employees to review and submit consent to receive
their annual 1095-C electronically through UKG.
1. Access UKG
Log in to your UKG account using your standard username and password.
2. Navigate to Government Forms
- Select the Menu icon (three lines) in the upper-left corner.
- Go to My Info tab.
- Select MY HR > Forms > Government Forms > 1095-C
3. Open the 1095-C Electronic Consent Form
On the Government Forms page, select the button at the top right titled Electronic Consent.
Check mark the box Receive Form 1095-C Electronically and then press Save.
Type your Full Name (exactly as seen listed below the box) and select the Confirm button.
4. Confirmation
After submitting your consent:
- You should see a confirmation message at the top of the screen.
The consent status will show as Consented under the 1095-C Electronic Consent section.
NOTE: To find this, visit My HR > Forms > Government Forms > 1095-C > Electronic Consent button and then you will see the consented message with date and time of consent listed below the checkbox.
- You will receive your 1095-C online when issued. Paper copies will no longer be mailed.
5. Changing Consent Later
If you wish to revoke your consent in the future:
- Return to My HR > Forms > Government Forms > 1095-C.
- Press the Electronic Consent at the top right.
- Uncheck the consent option and save your changes.
